Please note you may not need all Exhibits referenced; it depends on the basis of your eligibility to compete.
1. Proof of Citizenship: Copy of Birth Certificate, Passport, or Naturalized Citizenship Certificate
2. Proof of Age: Copy of Birth Certificate, Driver's License, Driver's Permit, State ID, or Passport
3. Eligibility to Compete:
*IF CLAIMING ELIGIBILITY AS AN IOWA RESIDENT: (1) Copy of Driver's License or State ID AND (2) Copy of mortgage, lease, utility bill, tax filing, etc. to document proof of residency. See items listed in contract section 2.4.1.
*IF CLAIMING ELIGIBILITY AS AN FULL TIME STUDENT IN IOWA: Copy of Registrar information to document enrollment showing full-time status, transcript for classes to date, OR degree awarded, if applicable.
4. Social Media Agreement: Submit Exhibit A, signed as required.
5. Health Condition and Request for Accommodation and Surgeon Certification, if applicable: Submit Exhibits C and E with documentation, if applicable.
6. Acknowledgment of Understanding for Miss America's Teen Scholarship Awards: Submit Exhibit F, signed as required.
